Referral Information

This page is for pediatricians, providers, and other professionals referring a child to Children’s Behavioral Medicine Collaborative. Below is how to send a referral, what to include, and what your patient can expect.

How to Send a Referral

What to Include

To help us route the referral and prepare for the family, please include:

  • The reason for referral and your primary clinical question
  • Relevant records, such as recent notes, prior evaluations, and pertinent medical history
  • The patient’s and caregiver’s contact information
  • Your contact information and preferred method for follow-up

What Happens After You Refer

Once we receive a referral, our team reaches out to the family to discuss the concern, determine the right type of service, and schedule. Families do not need a referral to begin, so a referral simply gives us the clinical context you are seeing.

What Your Patient Can Expect

Children’s BMC is not contracted with insurance companies. We provide superbills that many families submit for out-of-network reimbursement, and pricing varies reflective of time and complexity. Coordinating Care and Reports

Families receive a detailed written report and recommendations. With appropriate consent, we coordinate with your office.
